By December 31, 2021, LHC Group's operational footprint included 557 home health facilities, 170 hospice sites, 136 community-based service centers, 11 long-term acute care hospitals spread across 12 locations, and 14 Healthcare Innovations facilities. Established in 1994, the company's headquarters are situated in Lafayette, Louisiana.
